SF-50: A Standard form for Notification of Personal Action


An SF-50 is a specific Standard Federal form. More specifically, the SF-50 form is the form for Notification of Personal Action. This form can be used by a former or current Federal Employee to show proof of any Federal service or civil Service Status. The SF-50 can be found either at an agency’s human resource office for a current employee or at the National Personal Records Center for former employees.
